Guest Media Release
Please read the following media release for your upcoming interview and complete the acknowledgement section at the bottom to indicate your agreement. We'll send you a copy for your records when you submit your response. If you would like a copy prior to submitting your authorization, send an email to info@hackinghypermobility.com and we'd be happy to email you a copy.
1. Authorization
I hereby grant Two Bendy Mommas, LLC, producers of the Hacking Hypermobility Podcast (“the Podcast”), and its representatives, the irrevocable right and permission to:

Record my participation in audio and/or video format;

Use, publish, edit, and distribute these recordings in whole or in part;

Include my name, voice, likeness, and biographical material in connection with the Podcast episode(s) and related content.
2. Purpose and Distribution
I understand and agree that my appearance may be used for purposes including, but not limited to:

Podcast episodes distributed on platforms such as Spotify, Apple Podcasts, YouTube, and others;

Promotional materials, including social media, email newsletters, and digital or print advertising;

Educational or awareness-raising content related to hypermobility, chronic illness, or disability advocacy.
Execution of this Agreement does not obligate the Hacking Hypermobility Podcast or Two Bendy Mommas, LLC to broadcast or publish my interview, session, or any related materials.
3. Ownership, Rights, and Release of Liability
I acknowledge that all recordings and derivative works are the sole property of Two Bendy Mommas, LLC. I waive any right to inspect or approve the final product and any accompanying materials.
Guest releases the Hacking Hypermobility Podcast and Two Bendy Mommas, LLC, including their team members, affiliates, and anyone acting on their behalf, from any and all claims or liability related to the recording, production, distribution, or use of the session and related materials in any format.
4. Compensation
I understand that I will not receive compensation for my participation or for the use of my likeness and recordings. The Hacking Hypermobility podcast is an entirely independent, self-produced podcast run by three disabled women, without any financial backing or monetization. We will not profit from your interview appearance in any way.
5. Consent and Legal Capacity
I affirm that:

I am over 18 years of age,

I have read and understood this agreement,

I consent to the use of my likeness and recordings as described above.
I understand that any information I choose to provide, including personal identifying, biographical, or private health information, is shared voluntarily and at my sole discretion.
If the participant is under 18, this agreement must be signed by a parent or legal guardian.
6. Warranties and Indemnification
Guest affirms that during the course of the interview and/or session, Guest has not violated the rights of any third parties, including but not limited to copyrights, rights of privacy, protected health information, trade secrets, and non-disclosure agreements.
Guest further agrees that in the event of any breach of these warranties, Guest will indemnify, defend, and hold harmless the Hacking Hypermobility Podcast and Two Bendy Mommas, LLC, along with their affiliates, representatives, and assigns, from any claims or liabilities arising from such a breach.
